Use this checklist to track every document you need to prepare per criterion. Check off items as you secure them. Leave notes in the "Status / Remarks" column.


GENERAL / ADMINISTRATIVE REQUIREMENTS

Criterion C – Research-Related & Mentorship Services (max 10 pts)

You can only claim CSR points for extension projects also declared under Criterion B. Projects not listed there are not credited here.


Criterion D – Administrative Designation Bonus (up to +20 pts)

Inherent Duty Firewall: Accomplishments that are inherent to your designation's responsibilities (e.g., Dean revising curriculum, Director managing a project) cannot be claimed as separate IM, research, or extension project points.


KRA 4 – PROFESSIONAL DEVELOPMENT

Criterion A – Professional Organization Involvement (max 20 pts)

Important: If you earn a qualifying doctorate and receive the automatic +1 sub-rank, you cannot also claim the 20 pts for it under this criterion — choose one or the other.

B2. Trainings / Seminars / Workshops

Virtual webinars and virtual conferences are not credited. Only structured virtual training programs count. In-house SUC trainings are not credited unless the resource person comes from outside the SUC.

Paper presentations in virtual conferences are not credited. A paper later published in Scopus/WoS earns points under both this criterion and KRA 2 Criterion A.


Criterion C – Awards and Recognition (max 20 pts)

Only the highest award from a category is credited — placers, runners-up, and consolation prizes are not credited. Service awards and longevity awards do not qualify.
